Hormone replacements prescription letter

This is a template letter to request longer prescriptions for your essential non-steroid hormone medications.

Having longer prescription lengths (at least three months) for your essential hormone replacements is important to help you manage your health. This letter can be used to ask your GP to increase the length of your prescriptions, if you do not currently have three-month prescriptions.

What this letter does

This letter asks your GP to increase your hormone replacement prescriptions to at least three months.

Who this letter is for

You can use this letter if you have a diagnosed pituitary condition that is treated with any of the following:

• Desmopressin
• Levothyroxine
• Oestrogen
• Testosterone
