Adrenal Insufficiency Action Plan

This action plan is for people with any type of adrenal insufficiency or Addison's disease.

This leaflet can be used to record all the important details of your condition and care needs, such as:

• Your GP and endocrinologist details
• What medications you take, how much and when
• What adrenal insufficiency is
• The Sick Day Rules
• Signs of an adrenal crisis and what to do if you have one

This action plan can be given to others to inform them about your condition and what to do in an emergency. You may want to give copies to your GP, friends, family, people at work or other people you see often.

This leaflet was created in collaboration with the Addison's Disease Self-Help Group and is endorsed by the Society for Endocrinology. It was created in February 2026.
