Leaflet Order Form

We provide our leaflets free-of-charge to the community, however, a donation provided with your order form would be gratefully accepted. 

Please note: Leaflet orders from hospitals and clinics will be charged a postage and packing fee and there is a limit to the number of leaflets you can order:

  • Condition specific leaflets, i.e., Acromegaly, Cushing’s, Prolactinoma, Diabetes Insipidus (limit 10 per order)
  • General leaflets, i.e., Patient Factfile, Pituitary Gland and Its Hormones, Your Journey, etc. (limit 25 per order) 
  • Hydrocortisone for patients (limit 50 per order)
  • Hydrocortisone for parents (limit 25 per order)
  • Any orders over these amounts will be charged £1.00 per leaflet over the limit, plus postage and packing
  • Limit of four orders a year per clinic (unless Endocrine Clinic Member)

Please note: Within reason, these limits do not apply to Endocrine Clinic Members of The Pituitary Foundation.
